Populate a PDF from variables in a submitted record producer

Hi,

I have a requirement to populate fields on an already existing PDF when the record producer is submitted. We do not need a PDF newly generated. The above image is our record producer in the Service Portal, and the PDF we would like to have populated on submission. Along with populating the PDF with the values entered for the record producer, we would also like the PDF to be attached to the request after it is populated. 

After some research, people have suggesting using a script include called 'generalPdfUtils', but our database does not have that script include, nor do we have the associated application/plug-in installed. This is something I can't go ahead and just pay for and install and I won't be able to get this approved by my organization so I will need to be able to accomplish this without purchasing any license or plug-ins. 

Can someone let me know if this can be accomplished? To be able to populate an existing PDF when a request is submitted and have the PDF attached to the request.
