Are the priority look up rules correctly set?

The priority lookup is set correctly

However, recently I had included the Priority matrix for Request Item table(sc_req_item),   I suspect this could be the reason for the Incidents priority not being working

Can you deactivate the Priority matrix for Request Item table(sc_req_item) and see if it helps?
