Query regarding HR Document not loading for e-signature

I am updating 3 HR document template in PDF template field via business rule based on certain condition. This means, --when field1 value is 1 and field2 value is 2, update PDF template to one HR Document template
-when field1 value is 3 and field2 value is 4, update PDF template to second HR Document template
-when field1 value is 5 and field2 value is 6, update PDF template to third HR Document template
The BR is updating the PDF template field correctly.
Once, it is updated, a HR task will be generated for e-signature. Now when subject person i trying to do e-signature, the document is not getting loaded. It says document could not be loaded, please contact system administrator.
This issue is intermittent. If i create back to back cases, out of 5, 2 cases, document will be loaded, rest 3 it wont be loaded.

If it is very intermittent , better log a HI case.

Sure, will do.
So what I noticed is if I am able to sign one document for scenario 1 which is lets say: when field1 value is 1 and field2 value is 2, update PDF template to one HR Document template.
Now the very next time when i trying the sign by opening a case with same scenario as previous, I am unable to sign. It says document is not loaded.
But at the back-end we can see the PDF template value is updated correctly. Its just when employee tries to sign in esc portal, its not getting loaded.
Do we need to raise a HI for this as well?
