How can I raise incident using service portal ?

Yes, You can add "open incidents" into a different page and add that page into "icon link" widget.

Hello Gagan,

          You can create a record producer and add it to any category in the service catalog. The users can create incident using the record producer from service portal

Thanks for the response ,
 1. what if I want the same form which is available for raising incident OOTB?
2. If 1st condition is not possible then how to map all the fields within record producer , and how can I ensure that new incident which is raised using portal have unique incident number in a proper sequence .

Hi Gagan,

 

There is an OOTB record producer available in serviceNow as "Create Incident". you can add the record producer into your page.

Add "icon link" widget to your page-->Edit-->Then check the screenshot below.

Got the first point but able to add that item in portal page .
