Reference field is only showing "empty"

I created a table and see the records.  But on the form in the field, I do not see the records.  It only shows "Empty".  But when I select one it actually puts the name of that record in the field even though it shows empty in the list.

Adjust the form layout and also check the Display field on your Provider and NPI's table
