Regarding form employee centre

Hi Team,

     How to show a single from custom table based on logged in user in form view not in list view in employee centre service now. I have tried using simple list widget, but the client is not accepting it. They don't want the list view in simple list. They want to directly show the record. Please help me to achieve this. Because one user will have only one custom table

Org id is employee number? and a user will search from org id for own ticket or any one esle as well?
