Report with Approval and Variables for Requested Items.

I want to create a Report with Approval and Variables together for Requested Items. 

Approvals details should be some fields from sysapproval_approver table. Requested Item details sc_req_item and the Variables details including the Question and Value provided by User.

May I know what is use case where you need approval and Variable at one place. expert @Mark Manders  said you need DB for this.
