Restrict Survey to be only taken once from URL pasted in email Notification

I have made a survey with Survey Designer. I have also made an Outbound Email Notification to send a custom email with the URL of the survey posted into that email. How can I make sure when someone clicks the link from that email after they have successfully submitted the survey they are redirected to a page that says they have already submitted the survey?

Generally the surveys can be taken only once.

how it works is once a survey gets assigned a new assessment instance gets created and assigned to somebody. Once taken it gets closed .

 

@Community Alums the issue is whenever I use a Trigger in the survey to automate the email being sent when conditions are met, the link works as expected. Meaning whenever I go to the Email Outbox view and Preview Email of the email sent from the Survey Trigger, the link will open the correct survey, and if I try to open the link again after a successful submission it will prompt me I have already completed the survey.

My problem is I have made a custom Notification that is an Outbound Email and I have pasted the Survey's URL into this email. Whenever I go to the Email Outbox view and Preview Email of my Notification to click the survey link, it lets me have unlimited submissions. 

So in summary OOB Trigger email works fine, but my question is for manual distribution via pasting survey URL into my Outbound Email Notification the link lets me submit multiple times when I need it to only submit once, and give me a prompt I have already submitted/completed this survey when I try to click on the link again after a successful submisison.
