Risk Assessment on a table separate from change

I have a request to create an Enhancement in take request. Part of this Enhancement in take request is to add a Risk questionnaire similar to what is in the Change table. I know where I can create new risk assessments in the change table, but I cannot see away to create a risk assessment for a different table. Is there a way to add a risk assessment to a different table?

No, the risk assessment is come with plug in of change only and it is as per design, I dont think , we can use risk assessment  table for any other purpose.
