Risk Not Updating from "Fill Out Risk Assessment" in Change Request

Hello all,

I clicked the "Fill Out Risk Assessment" UI action in the Change Request and completed the assessment. A record was successfully created in the task_assessment table. However, the risk field in the Change Request was not updating based on the assessment score.

Could anyone help me troubleshoot this issue?

Hi @aravindk591 

Is this something custom you've built, or are you using the OOTB (Out of the Box) Risk Assessment UI action? Also, the assessment is created in the Assessment table. If it’s OOTB, then... try to repair the risk assessment plugin.
