RITM stuck on "Requested" even though its been approved

Hi Team,
I am facing an issue with an RITM workflow. Even though the RITM approval is approved, the approval state shows as Requested. The workflow is not moving to the next stage (SCTASK creation).
Expected Behavior:
Once the RITM approval is approved, the workflow should automatically transition to the next activity and generate the required SCTASK.
Actual Behavior:
The approval record is approved by approver, but the workflow remains stuck on the approval activity(Requested) and does not proceed further.
Troubleshooting Done:
- Verified workflow transitions and conditions.
- Checked approval records in sysapproval_approver.
- Manually used “Nudge”, and it proceeds correctly.
- No errors in system logs during approval update.
Could you please suggest what additional configuration or workflow trigger I may be missing to ensure the workflow progresses automatically?

Can you please share the Approval step details.
Are you approving as an admin or as the same user as approver?
Does the approver have any delegates defined?

Please find attached workflow. Highlighted is the exact flow.
Initially stage would be ''waiting for approval''. Once user approves the approval, stage should be changed to Approved.
Approval will be approved by User having hr_analyst role. But user doesn't have access to RITM record.
No delegates for approver.
