Weird issue...
We have a catalog template, built 2 99% identical catalog items. Price and name differ, to be able to differentiate.
One fires the flow just fine.
The other fails...
If I run a "Test" in Workflow Studio with the RITM that failed, interestingly it runs just fine and creates approvals etc.
In the screenshots you can see it. The RTItem with "Item 5" at minute 26, didn't fire an "workflow execution"
I went so far as to export the two catalog items to XML and check for differences. Literally the only differences are name/price/times/sys_id
I'm stumped.. should I report a bug?

turn out: there was a sysrule_approvals which didn't approve automatically all requests with > 1000$.
Basically costly requests were not auto approved hence the workflow on RITM didn't fire.

Have you tried Deactivating and reactivating the flow?
try it out and test it

didn't help... as I was expecting.. why would it work with some catalog items, but not with others...

I agree with @Chaitanya ILCR. Try to activate the flow again and run a few more test cases. Also, check the Service Catalog Builder plugin. If the issue persists, consider logging a new support case.
