unfortunately it will not possible to do it like this, it is possible to use the background (gradient pink-yellow), it will be possible to add the logo (but to the left upper corner only), the right upper corner is for the logged in user (their profile)
The icon will definitely not be possible, and the search bar is in the centre part in the top bar.
Please, go to https://developer.servicenow.com (using the same account as here) and request there your own instance, then go to Service Portal or Employee Service Centre and observe how these look like.
What you have in the requirement is like 90 % impossible to achieve in ServiceNow.
It would be easier to have a webpage separated from ServiceNow and the icons would be links to ServiceNow forms and features. But you will not get any close to the picture, not even with heavy customisations.

You can adjust many things but the overall template will be more or less like the screenshots that I just attached.
Can you tell me the business justification? Because it seems that the requirement is originating from someone who never saw how ServiceNow looks. :(( Let me know if you want to discuss something further.
