ServiceNow Azure DevOps Integration - incident table

Hi all,
I'm trying to build an integration between ServiceNow and Microsoft Azure DevOps using the out of the box spoke. I have set up the connection as you can see here, and it posts fine for the out of the box stuff like Epics. I am trying to set up the integration between Incidents and a custom table we have in Azure called Tickets. I have set up the connection/credential and completed the mapping. All of that has come back fine - I have imported work items/projects and have tested the connection, which works. I've also set up the team integration settings. However, it only works when I change the team name, not when I create an incident from scratch which is already assigned to that team. It creates the item in DevOps when the team name is changed though. The integration works fine there on from the DevOps side - any changes in DevOps sync back to ServiceNow ok. However, any further changes in ServiceNow are not syncing back to DevOps. So the update work item doesn't seem to be working, even though after checking the flows everything looks like it's executed correctly. Has anyone experienced this before and does anyone know what to check? I'm struggling, I've been stuck on this for months.
