servicenow survey configuration for reference record

How can I remove this reference record details from the survey when it is triggered.

Check these places :

A. Navigate to Survey ->View Survey

  Open your survey -> at right top corner  click on Survey Designer

Check if you are getting the place  in name , if yes modify it.  // this is for static value

B. 

1.Navigate to sysevent_email_action.list 

2.Identify your Survey invitation on Assessment Instance table 

3.Now Go to What it will contain tab 

4. Update message body -> remove the  number. 

C. Also check if  "assessment_take2" UI Page has been modified for same purpose.
