1.Navigate to sysevent_email_action.list
2.Identify your Survey invitation on Assessment Instance table
3.Now Go to What it will contain tab
4. Update message body -> remove the extra number.

Hi @Tanushree Maiti , I havent configured any notification for this survey. From asmt_assessment_instance we will be able to see the triggered assessment instance, even here when I'm checking here by opening the url I'm seeing the target record info on the survey.

Hi @RohitGeorgV ,
If there is no notification , in that case check
if "assessment_take2" UI Page has been modified for same purpose.
Update Incident Number and other incident information on Survey Page
