Why are there multiple entries for the same record when I click on Show All Approvals on the my approvals page in the service portal.

In the attached file you will see that once I had clicked the Show All Approvals, one change is listed 3 times, with "Approved" against one and blank against the other two. All records show a similar pattern.

It looks like a custom solution. Did you check if the change in the backend has one or more approvals, and whether the same is reflected on the portal?
