Show Delivery Address field in sc_task is condition is met

Hi Experts, 

Good day!
I want to configure that when the selected value of employee_location field is 'stateside'  from a variable set called user_data and added to the catalog item 'iPhone', it will show the filled out delivery_address from the checkout page into the Notes section of catalog task that it will generate as shown example below.  I have tried to look for existing client scripts that will affect the behavior of the shipping address but not found any related to it. Thus, I have created client scripts: 

Catalog task from different catalog item - Laptop Request to mimic: 

What the actual catalog task is generated using Catalog item - iPhone. 

We have created a catalog client script below but it didn't work. 

We have created also a client script below but it still not showing the delivery_address in the catalog task. 

I would appreciate any help regarding this matter.

is that a dot walked field on Catalog task form?

employee_location is variable right?
