Stages Not Showing Properly on Service Portal

I'm working on a catalog item that generates a REQ, RITM and SCTASK, and uses Flow Designer to handle the processing of the request. For some reason, this catalog item keeps getting stuck on the stage "Request Approval" instead of moving through and showing all of the correct stages from the flow. This issue is only happening on the service portal. In the platform view, it is rendering correctly. This issue has happened before on other catalog items, but it seems random and I'm not sure how to fix it or keep it from happening. How do I get the service portal to show the stages correctly for this catalog item? For the record, at the current moment this catalog item is the only one affected. The other catalog items show the stages as desired.

Note:

Step 3 in the flow updates the RITM to Approval Requested and State Open,

Step 4 in the flow updates the REQ to Approval Requested and Request State to Pending Approval.

Hello @kaylamillerdev ,

If other than the stages, everything working fine in service portal from the flow then check stage configuration in the flow, clear cache and check by refreshing service portal, check using other users if any user permission issues  and check other configuration done that are done from the time from when things looks wrong.

I appreciate your speedy response. I've already checked the stage configuration and it seems fine. I'm using the out of the box ones for Requests. No change upon refreshing the service portal. It's just this one catalog item that is affected. User permissions doesn't make sense for this scenario. The account I'm using has admin privilege.
