Standard Change not going through the Workflow when created via Record Producer

Hello everyone. My developer is on PTO this week and I'm trying to figure out a problem, but not sure where to look.
When submitting a standard change manually, it follows this process (which is correct)
1. New - Change is created
2. Schedule Phase
3. Implement Phase
4. Post Implementation Ctask is created
5. Ctask closes (manual), Automatically moves to Review Phase
6. Add closure notes, click close button, Change Closes.
We have standard change templates attached to Record Producers as well.
When these are submitted, the change process breaks.
1. Change is created from Record Producer
2. Schedule
3. Implement
4. NO Ctask gets created. You have to manually create one to move to Review phase
5. Close ctask, change state drop down to Review (doesn't automatically move).
6. Once in review stage, no Close button appears.
7. Add close notes, save record, Close button appears.
8. Close Change
Not sure why this started happening about a week ago.
But does anyone know where I should look to see why Record Producer created changes are not following process?

The first point is to check is right version of template has been mapped to Record producer.
2nd is any changes done in Flow/ workflow
By any chance any mapping done in RO with change model ( chances are less but need to see)

When looking further, it looks like it's not applying the workflow to the change request when submitted via Record Producer.

I think it has to do with these conditions, but I can't see the whole dot walk path..
Every time I click on it, it drops down, but doesn't show me what's selected.

Seems someone change in WF.
You need to check out the WF to see the conditions / dot walk.
