Standard Change Proposal on Poral page shows sysId

Hello All,

 

On Poral page, Approval tab show std change request and when I open that record, some of the fields show sys id.

But when I open same change request record on self-service page, it shows the actual value and not sys id. 

 

I have attached the screenshot of it for better understanding of the issue. 

I need some help to understand from where this value is coming from on portal page and how do I change it?
