Subcategory keeps disappearing after I save a record

We have Category and Subcategory in our incident form.  The subcategory is dependant on the Category 

Once you select Category and select SubCat and Issue and hit save the SubCat and Issue both disappear.

Steps I have taken: 

I have checked the dependant value and and it is correct to what is in subcat matches

Checked if there is a UI Policy and I have them set-up correct

No BR in place or client script either 

I added a video to show what is happening. 

 

Hopefully someone can assist - PLEASE

Check again if you have UI policy or BR or Client Script as it would be the potential cause.

 

Also see if you have both UI policy and Client Script as UI policy would over-ride Client Script. Refer below for more details,

If it still does not work, enable trace and simulate the scenario to get more details on why the field disappears.

Can you share a screenshot of your UI Policy (every relevant tab)? I am curious if something like the "Reverse if false" checkbox might be altering the visibility.

Hopefully you are able to see this..  I have unchecked the "Reverse if false" but it has still not worked
