Not even explained, but just a screenshot of the requirement? Without knowing what you already have, it's unclear what you need to do.

You will have to create jobs to collect the responses and a formula indicator to calculate. Put the breakdown on it and you are done.

If you don't know how to create jobs or indicators, I advice to go to NowLearning and follow a PA course.
