Surveys: How to add label in the Survey Question.

Hi Team,

I have added a survey question which in SNOW looks like below snapshot:

But I am looking for something like shapshot2 i need to add for surveys. Right now i am using a template and i don't have option to create a label for adding the question to above.

Let me know how to achieve this.

It is apparently not possible.

As a workaround I did create a new template type (Assessment Metric Template) with no definition

Then I use it adding a label.

But it is just a workaround 😞

Is there a way to display a field label along with its corresponding value on the survey form?
