Suvery pop for pending surveys with users

Hi Team

I have requirement where i want to pop up for surveys on service portal pending with users when user logged in ..surveys which collected in last 15 days for user and from pop itself user able to take up or reject survey with cross option i need solution on that .

Help will be much appreated

As per my knowledge here we can add a new widget within the widget we can add the code spModal but this would be customization.
