task_sla record created twice for single record

Hello All,
Good day!!
I have a transform map which create a record in custom table. The state for task is closed complete.
When transform is ran and record is created, task_sla for the record is getting attached twice.
SLA start condition - L2 service is TEST
SLA End condition - State is closed complete.
If we remove the SLA end condition, then it is created only once.
Has anyone encountered the same issue before?

Any business rule on your custom table has current.insert()?
