The attachment icon is missing in the incident form

The attachment icon is visible when the incident form is loading, but when the incident form is loaded completely it is gone missing. Can someone help in this issue please

Please check if no_attachement attribute is set @ incident table?

Hi Hari,
Can you check the attribute no_attachment on the collection field on incident table.
Also check any client script has code to hide the icon using DOM or g_form.disableAttachment().
