Trigger survey from email notification

Hello,

I have created a survey. Now I want to send the survey link whitin the notification to the user. So when the notification triggers it should have the link to survey 

I have created a notification in requested item table, also created email script where I'm adding the sys_id of the survey and created a button 

But none of the things are working like button and survey thing. Here is the email script I created 

Please someone help me how to proceed on this part

so what's not working?

did you add logs in that email script and see?

did the link generate properly in email logs?

I would suggest checking the existing configuration in platform which is already available and try to do same config as per your requirement.

In your instance or PDI append this URL where you will see notification "Incident Survey" available which is sent for survey with link.

Also, this above notification is triggered by event which is called from BR "Dispatch Survey Notification Event".
