Trigger survey from email notification

Hello,

I have created a survey. Now I want to send the survey link whitin the notification to the user. So when the notification triggers it should have the link to survey 

I have created a notification in requested item table, also created email script where I'm adding the sys_id of the survey and created a button 

But none of the things are working like button and survey thing. Here is the email script I created 

Please someone help me how to proceed on this part

This script is now working and link is also redirecting to the esc portal

The only problem now is it is not redirecting me to the respective survey 

 

Once we click on the link esc portal got opened, the goal is to open the survey in the esc portal and it's not happening
