UI action with a model that Cancels an incident

 

I need to create a UI action called "Cancel" in a way that whenever a user wants to cancel an incident, he/she can click on the UI Action and it pops up a model called Justify, so that the user can write some description, why he/she is cancelling the incident. The logic behind should move the incident state to cancel.

 

How can I achieve this?
