Unable to add CTASK in Change request (CHG0559385)

We have raised a Change Request (CHG0559385) today, but we are unable to add CTASK on that CR. we are getting an error (screenshot attached). we have contacted with change management team already, but they are unable do it from their end.
also, in one CTASK the state is disabled (screenshot attached).

can anyone helped me on this

@PRADEEPREDDY GU 

seems some before insert BR is stopping the insert. Did you check that?

Did you check any state model on change task is blocking?

Look like it is caused by Flow or some BR. Please check the code of the New Ctask button once.
