Unable to find the lookup option for the reference fields when the change is created from Incident

Hi 

 

When we are trying to create a change request from the Change request/Caused by change fields which are located in the related records tab of the incident form. The lookup option to select the data for the field on the Change request form are disabled for the reference fields. 

 

Attached the screenshots for reference.

 

Can someone help in resolving this.

Can you check if any customization is made for this I'm not seeing such issue in my personal developer instance.
