So the next time it happens you have to investigate the collision. The warning should appear as a line item on the previewed update set record. You can compare the local version to the version the update set is trying to apply. From there its a discussion with the person you have a collision with to see which version is most authoritative.

Well, if you committed the update set things will be a little more complicated.
Go to the update set record, click "Show All Preview Records", then filter so you only show Problem Type of "Error"
For each of those errors, look at the contents of the remote and local update sets, find out the sys_updated_by and start reconciling which version should have been authoritative.
