we are already using 3 varible set 

Product numbers and options variable set , I have to show based on type of request . 

how to do that

It is Multi-row variable (MRVS) sets. You can find out in Catalog item Variable sets related lists.

And this multirow variable set has to be visible only when type of request is FMS Totals Rebuild

In the variable set i have given only 3 variables 

IDK why in the catalog form 4 variables showing

No its not showing 4. I see the Actions column which is on every Multi row variable set + your 3 variables. Exactly as how you designed it.
