Walk-Up Experience "Schedule an appointment": How to add fields here?

Hello ServiceNow community,

i need to add some custom fields to the "schedule an appointment" tab like 3 fields which are text fields. How can I achieve that?
I saw some threads where some people talked about cloning the widget and customizing it, is this right and is this the only way?

Im in the process of trying to figure out how to add fields as well, referencing this post I was successfully able to copy the widget and have it render on the page correctly, just have to figure out how to add the extra fields.  As far as I know/Have read that is the only way to accomplish the task of adding custom fields.
