we have a survey called IT satisfaction Survey.
Which would be triggered when an incident or request gets completed.
When the survey is triggered for an Incident the tile has the Short Description of the Incident.
But the same is not happening for the Survey triggered by Request Item.
Can someone please tell me where i can able to change this

Hey Abdul,
I think OOTB the short description field is not shown on the RITM form, so most probably it's blank, hence you are not seeing it in the survey

To verify, you can navigate to the RITM in question and view the xml to check if the short description is empty.
Also, to test you can add it to the form and fill it out and most probably this will fix your issue.
To answer your original question you can navigate to Survey > View Surveys, find the survey in question and click on the Survey Designer button on the form

Hi thank you for this link but i am still unable to get the short description to be displayed on the Request item survey.
