Does the notification otherwise appear correct, only it is missing every, or only one/some of the CC addresses? Does anything appear in the system logs when this is notification is triggered? Are all of the email addresses and names exactly correct? Have you tried with only one cc address? I really hope this doesn't matter, but I've seen stranger unexplainable things - try enclosing each cc in double quotes instead of single.
