Hi Priyanka
Is your trigger "Service Catalog"?
In order to make a flow for a specific Requested Item, you should first create a catalog Item selecting the appropriate category. Once your catalog item is fully created, you create a flow which you attach to the specific catalog item.
Each time a user "orders" a catalog item, the flow is triggered.
Your steps could be
request manager approval
create catalog item task (optional wait to close)
send notifications
fulfillment activities etc and then logically end the flow.
Which part are you having trouble with? can you share a picture of your flow?

It's OOTB behavior and you cannot create flow with trigger on sc_req_item table
This KB confirms the same

Once you choose trigger as Service Catalog on the right hand side data pill will have requested item already,
