Widget to get a record producer on Service Portal

How can I get a record producer on Service Portal? Like can anyone explain how to accomplish this?

hi DVP and Chris,

What I am trying to achieve is a simple record producer(Shown below)which should be displayed when a user clicks on 'Report an Issue' widget.

There will be a submit button down of the form, when clicked Submit should create an Incident.

Here is the sample of what we implemented

When user clicks on it, it will take then to the record producer page

we implemented using icon link widget.

Kind of similar I am looking at. Instead of taking to the different page, When you click on report an issue, the record producer should be seen under report an issue. Am I clear ?

For the button, set the instance options: change the link type to catalog item, and then in the reference field select the record producer.
