Workflow not attached in RITM so it is not moving to approval state.

Hi
I have recieved a RITM which is in open state even after getting CO-ID it is not moving to approval state, I saw in related links to check the workflow but there was no workflow attach and workflow is present and mapped to Catalog item.
How can I attach workflow to the RITM and why this occurred suddenly .
Please provide fix script to attach the workflow from background.

This happens sometimes although workflow is still executing in background to see that you can go to wf_context.list table and find your RITM there where you can see your attached workflow
