Workflow not triggering

I have a simple catalog item request for an ITIL license through the catalog. I have a workflow that sends approval requests once it's approved it should spawn the tasks. I checked the executions on the workflow and the request isn't even hitting it. I'm not sure what went wrong it's been working fine for over a year and now in the past couple of weeks it stopped triggering the workflow. I can't think of anything else to check.
Side note I can rerun the requested item through the workflow manually and it works fine.

I ended up recreating the flow and it seems to be working now, but i'm not sure how it got corrupted to begin with. That item and flow haven't been modified at all for almost 9 months. The only change is I added another approver but not sure that's what caused the flow to stop working.
