Workflow not triggering

I have a simple catalog item request for an ITIL license through the catalog.  I have a workflow that sends approval requests once it's approved it should spawn the tasks.  I checked the executions on the workflow and the request isn't even hitting it.  I'm not sure what went wrong it's been working fine for over a year and now in the past couple of weeks it stopped triggering the workflow.  I can't think of anything else to check.

 

Side note I can rerun the requested item through the workflow manually and it works fine.

Do you know why it would suddenly stop working?  I mean it's fixed, because I created the flow from scratch again, but why did it stop working?

I can't say on it , as it's very subjective to specific flow. There may be some internal parameter object corrupted.
