Worknote is updated twice on incident form

In incident form whenever I update the Assigned to field or one field is Escalated code then in worknote it is updated twice. I am not getting what is the reason for this. So, please suggest where the issue is.

Can you check if there is any After Update Business Rule in which the above conditions are getting matched causing it to update multiple times.

Watch out for current.update() in a business rule.
