catalog item not working in employee center

Hi All,

We have recently implemented employee center.

One of the catalog not working , means not showing any questions in employee center but working fine in old portal and in mail catalog view.

Please let us know what can be issue.

@Roopa11 On your catalog item variables, can you check if the following checkboxes are set to checked.
