When to declare a major incident. Why do people hesitate?

Recently, my organization has experienced delays in standing up the major incident process. One recurring reason is employee reluctance to "make the call". I am going to write an article to share with my organization. I am looking for input and guidance from this group. Have you experienced similar events? How have you addressed the issue?

I would entrust the duty of "Promoting" with a few people in which absolute trust is given.
"Bob, Jane, Mohammed... its YOUR call"
Pressure is off of all others... who should feel free to mark incidents as CANDIDATES whenever they please (that's OOB functionality).
People hesitate taking it over that threshold because it usually means so much interrupted work because of the political scramble. Small number of empowered users resolves that (usually)
