Opened date and created date is having a difference of more than 4 months

i do understand opened field is set when the form is loaded for the first time and created filed when the incident is created but in my case I have 2-3 incidents whose created and opened difference is more than 4 months how this is possible?

Please help to understand

Following reasons could be the possible explanations.

1. Opened date was changed deliberately by users 

2. Incident might have a parent incident which led to oversight of the child incident

3. No one cared to check those incident records
