Your first lookup record action is referring to CMDB Health Result(guessing it as the images are quite blurry).

Are you referring the Health Result record's sysid in the 2nd step? If yes, then it wont find any record.

If there is any field on CMDB health result table which refers to cmdb_ci table then you can use that field to dot walk its sysid
