UI policy not working for custom field

I'm trying to get two custom fields i built on the change request form to show up o their respective change task. however the ui policy isn't working and still showing both fields on the the task. 

i put a condition for the type to be either implementation or verifier and the field to be "false" for visible. 

its still showing on both task

am i missing a condition?
