Catalog Task not getting created on submitting Hardware Asset Refresh Order catalog item

Hi

I am trying to test the Hardware Asset Refresh Order catalog item but it is not generating catalog task for RITM. Based on the service now documentation, the Source Request workflow should execute create a catalog task. The Source Request workflow just has the Create Task node (and Start and End). This is all OOB. Why is catalog task not getting created> Any pointers?
