OOTB, Risks gets created when you add a Entity type to the risk statement for the respective entity.

All Risks can be found under Risk Regiter> All Risks

All of the Risks are mastered in "sn_risk_risk" table.

Risk cannot be conveted as Issue. However, Issues can be created as part of Risk Lifecycle.
